Would you consider tying or chaining your puppy inside your home? Outside the home? Do you have a securely fenced yard? If not, please describe how you would make sure your puppy will be provided with adequate exercise and times to relieve itself. Will your puppy be raised as a member of your household? Do you understand that the puppy purchased from us will receive a limited registration and is placed with a spay/neuter contract? The registration papers will be released to you when we have proof from your veterinarian of the spay/neuter. Do you understand that this dog must be returned to Happy Havs Farm with signed registration papers at your expense if: 1) you are unable to care your puppy any longer 2) you no longer want your puppy 3) you violate the written contract signed at the time of purchase 4) unforeseen tragedy would leave your puppy homeless 5) you are no longer able to maintain your puppy in the environment it has been accustomed to and feel he is in distress or distressed Reply: Do you understand that this questionnaire will be made part of the written contract at the time of purchase?
